1. Pre-Visit Paperwork
Typically, we dispatch this to you ahead of your appointment. If feasible, mail it back to us. If not, simply bring it along.
2. Medication List
Do prepare a list of medications you’re currently on.
3. Dental Insurance Details
If you’re insured, don’t forget your insurance cards or related forms. This helps streamline the process.
4. For Our Young Patients
Kindly note, if you’re below 18, you need to be accompanied by a parent or guardian.
5. Medical Heads-Up
If there are existing medical conditions or concerns that might affect your treatment (like artificial heart valves, particular heart murmurs, extreme diabetes, or hypertension), please give us a heads up in advance.

Tuesday to Thursday: 7am – 4pm
Friday: 7am – 3pm
